Care Management Referral 2024

To be completed by agencies who serve families who are also served by YFC. Staff can complete the referral form to request funding. Funding requests will be approved/denied after staff are scheduled to present to Care Management on behalf of the family.

Funding categories included, but not limited to;

* Transportation (gas vouchers/bus passes)

* Respite (Including summer camp) 

* Direct Parental Placement (upon children services recommendation)

* Non-clinical Behavioral Health Services (that are not covered by insurance) 

* Social/Recreational Activities (YMCA Memberships, Art Classes, Etc) 

* Safety and Adaptive Equipment
